The Fatty Liver Index (FLI) is a simple and accurate predictor of hepatic steatosis (fatty liver) in the general population. It uses common clinical and laboratory measurements to help identify individuals who may have fatty liver disease, even before more invasive testing is performed.

What is the Fatty Liver Index?

The Fatty Liver Index (FLI) is a mathematical formula developed by Bedogni et al. that combines four parameters:

  • Body Mass Index (BMI)

  • Triglycerides
  • Gamma-glutamyl transferase (GGT)

These parameters are used to generate a score from 0 to 100 that indicates the likelihood of having fatty liver disease. The FLI was developed and validated in a study of 496 subjects from the general population of Northern Italy.

The FLI score ranges from 0 to 100, with the following interpretation:

  • FLI < 30: Rules out fatty liver (negative likelihood ratio = 0.2)
  • FLI 30-60: Intermediate probability of fatty liver
  • FLI ≥ 60: Rules in fatty liver (positive likelihood ratio = 4.3)
